Insurance Verification for Treatment at Marin Acupuncture Clinic
Please fill out the verification form below prior to making an appointment or call us with your insurance information. We will get back to you within 1-2 business days with your coverage information (i.e. number of sessions that will be covered, co-pay and deductible amounts, etc). If you are a Veteran we will need you to fax/email us your VA/Triwest Authorization as well. If you have a Worker’s Comp and Car Accident case, please include your adjuster’s name/phone and your claim number.
We are in-network with Blue Shield PPO (Federal and Medicare Supplemental plans only) and TriWest/Veteran’s Choice but an out-of-network provider for all other plans. Please note that Kaiser and HMO plans will not cover our services. If you do not have insurance coverage for our treatments, Health Savings Accounts and Flexible Spending Accounts (HSA / FSA) can be used instead to pay for our sessions (do not require verification, as acupuncture is fully approved).
